Contact your regular doctor or the OK State Department of Health for locations to obtain immunizations your child may still need.

Vaccines required to enter school (K-12)

  • Hepatitis B.

  • Diphtheria, tetanus, pertussis (DTap).*

  • Inactivated poliovirus (IPV).*

  • Measles, mumps, rubella (MMR).*

  • Varicella (chickenpox).*

  • Tetanus, diphtheria, pertussis (Tdap).**

*Students 4 through 6 years of age are required to have their final doses of DTaP, IPV, MMR, and Varicella prior to kindergarten entry.

**Students are required to have Tdap prior to entry into 7th grade. One dose of Tdap is required for 6th through 12th grades.

Immunizations Exemption

Oklahoma law allows exemptions for medical, religious or personal reasons. Students may have an exemption to one or more vaccines and receive the other vaccinations.  In this case a student may have an immunization record and an exemption certificate on file with the school. To apply for an exemption, please fill out the exemption certificate and bring it to your school.
